CASE MANAGEMENT

 

Case Management services are designed to allow the client to successfully access resources through a strengths based model in their respective communities. CM provides resources and information the consumer as requested or identified in service planning. Direct case management services are those performed face to face with the child, parent/guardian, or other service provider necessary for the implementation of activities delineated in the treatment plan. Indirect case management services are phone calls, monitoring of clients progress and the case manager’s travel time to and from activities necessary for the implementation of the treatment plan. Other indirect services may be communication through letters, or memorandums to treatment or other service providers necessary for the implementation of activities delineated in the treatment plan. CM is offered to all adults and children who have substance abuse/dependency disorders and to their family members. CM credentialing required.
